This is actually pretty simple to make. If you have an espresso machine or espresso pot, so much the better, but it's easy enough to use strong brewed coffee.
The real trick with this is to get the right proportion of milk to coffee. With espresso, it's about 1 part espresso to 2-3 parts milk (depending on your personal taste). With strong brewed coffee, it's about 1 part coffee to 2 parts milk. You may need to experiment a little to get it the way you like it.
You will need:
Espresso or STRONG brewed coffee
Milk (whole is preferable, but skim will work if you're watching your fat intake)
Ice
Optional: Flavored coffee syrup. My favorite is vanilla, but you can experiment and see what you like best.
Optional: Sugar or other sweetener (but probably not along with the syrup)
Fill a tall glass about half full with ice. Pour in milk, leaving enough space to put the coffee in. Add hot coffee. Note that the ice will mostly melt, and if it's an actual GLASS, be sure to put in a metal spoon to absorb the extra heat and keep the glass from shattering. Add sugar or syrup as you like. Stir well. Sip and say, "Mmmm, this is nice."
Another good option is to use a scoop of ice cream (I prefer vanilla, but chocolate is really nice, too) in the glass in place of the ice. Sort of a "coffee float".
